Privacy Policy
Axial Chiropractic
Axial Chiropractic is committed to protecting your privacy and handling your personal information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs).
What Information We Collect
We may collect personal and health information including, but not limited to, your name, date of birth, address, phone number, email address, relevant health information, medical history, clinical notes, appointment details, billing information, and audio recordings used for clinical documentation purposes where consent has been provided.
Why We Collect Your Information
We collect your information to provide chiropractic care and related services, maintain accurate clinical records, communicate with you regarding appointments and care, and meet our legal, regulatory, and professional obligations.
How We Collect Information
Information may be collected directly from you through intake forms, consultations, or verbal communication, and through electronic practice management and documentation systems.
Use of AI-Assisted Documentation
Axial Chiropractic may use AI-assisted software to support clinical documentation. Where used, audio recording may occur during consultations. All clinical notes are reviewed, edited, and approved by your practitioner. AI systems do not make clinical decisions or provide treatment. Use of this technology is subject to your consent.
Storage and Security
Your information is stored securely using reputable electronic practice management systems and cloud-based services. We take reasonable steps to protect personal information from misuse, loss, unauthorised access, or disclosure.
Disclosure of Information
We may disclose your information to other healthcare providers involved in your care, to service providers that support our practice operations, or where required or authorised by law. Some service providers may store or process information outside Australia. We take reasonable steps to ensure overseas recipients handle personal information in a manner consistent with Australian privacy laws.
Access and Correction
You may request access to or correction of your personal information by contacting Axial Chiropractic. Requests will be handled within a reasonable timeframe.
Privacy Complaints
If you have concerns about how your personal information has been handled, please contact Axial Chiropractic. We will respond to privacy complaints within a reasonable time. If you are not satisfied with our response, you may contact the Office of the Australian Information Commissioner (OAIC).
Fees, Payments & Appointment Policy
Axial Chiropractic
This policy outlines our fees, payment expectations, and appointment management to ensure clarity and
fairness for all patients.
Fees and Payment
Payment is required on the day of service. We accept EFTPOS, credit and debit cards, and other payment
methods made available at the clinic. Fees will be explained prior to treatment where possible.
Private Health Insurance
If you have private health insurance, rebates may be available depending on your level of cover. Claims may
be processed on the day using electronic claiming systems where available. It is your responsibility to confirm
eligibility and rebate amounts with your health fund.
Medicare and Other Schemes
Where applicable, Medicare or other third-party schemes may be used in accordance with their rules and
referral requirements. Any out-of-pocket costs remain the patient’s responsibility.
Appointment Cancellations and Missed Appointments
We require at least 24 hours’ notice for appointment cancellations or rescheduling. Appointments cancelled
with less than 24 hours’ notice, or missed without notice, may incur a late cancellation or no-show fee. Late
arrival to appointments may result in reduced treatment time or the need to reschedule.
Refunds
Fees are charged for professional time and services provided. Refunds are not routinely provided once a
service has been delivered. Any concerns regarding fees should be raised with the clinic as soon as possible.
Changes to Fees or Policies
Axial Chiropractic reserves the right to update fees or this policy from time to time. Any changes will be
communicated where appropriate.
If you have any questions about fees, payments, or appointments, please speak with us — we are always
happy to clarify.
